What is Telecare Virtual Geriatrics Model?
Seymour Health's residential in-reach program is utilising Telecare Virtual Geriatrician Model at Barrabill House. The co-design model is to deliver better access of geriatrics care to the aged care resident, facilitated by the in-reach program team. By introducing the virtual geriatrics services, the HHSP in-reach program aims at providing more holistic clinical care with specialist support.
What to expect
As part of the Virtual Geriatrics Program, patients should expect a level of care consistent with the same quality standards as in-person services. As part of the program, patients must consent to being looked after virtually.
Upon the request for geriatrics assessment being initiated by the in-reach coordinator, Telecare geriatrician will connect with the patient at Barrabill House with the in-reach coordinator via our telehealth portal.
The in-reach program clinicians communicate with Telecare geriatricians through the Telecare portal. Patient care will not be compromised by having a doctor available virtually, and it allows residents to be treated closer to home in a more timely manner.
